Snohomish County Aging and Disability Services has announced the availability of expanded support services for eligible residents enrolled in Apple Health (Medicaid) with the Health Related Social Needs (HRSN) Program. The services are expected to be available through June 30, 2028.
These free resources are designed to improve health, safety and housing stability for qualifying Apple Health members experiencing food insecurity, behavioral health conditions and/or chronic illnesses across Snohomish County. The HRSN Program connects eligible individuals and families with services that address barriers impacting overall health and well-being, including food security and home safety needs.
Apple Health enrollees experiencing food insecurity may qualify for up to six months of healthy food support. Available services may include fresh fruits and vegetables, grocery assistance for individuals who are pregnant or under the age of 21, medically tailored meals designed to support specific health conditions, and ongoing nutrition education services upon request.
Eligible Apple Health enrollees living with behavioral health conditions or chronic illnesses may also qualify for health- and safety-related home support through the program. Assistance may include air conditioners or heaters, refrigerators for food or medication storage, air purifiers, backup power supplies for medical devices, accessibility improvements, such as ramps or grab bars, and minor home modifications that address safety concerns.
“These services are intended to help residents remain healthy, safe and stable in their homes and communities,” said Laura White, division manager, aging and disability services. “Addressing health-related social needs can reduce barriers to care and improve long-term health outcomes for vulnerable community members.”
